• All owners’ Driver’s Licenses (front and back)
• IRS Form 940 and 941 (as of 12/31/19 and 3/31/20, respectively)
• Payroll Report for closest date to February 15, 2020
• Year-End Payroll Report for 2019 (calendar year through December 31, 2019)
• If you received an SBA EIDL loan since 1/31/2020, provide a copy of the note
